Heart Failure Appropriate Care Measure (ACM)

Heart Failure (HF) is a weakening of the heart’s pumping power. With heart failure, your body doesn’t get enough oxygen and nutrients to meet its needs. The Heart Failure ACM is comprised of 4 measures that reflect standards of care for which there is national support and that have been selected by CMS for public reporting.

The four Heart Failure measures are:

HF-1	Discharge Instructions
HF-2	Left Ventricular Systolic (LVS) Function
HF-3	Angiotensin Converting Enzyme Inhibitors (ACEI) or Angiotensin Receptor Blockers (ARB) for Left Systolic Ventricular Dysfunction(LVSD)
HF-4	Adult Smoking Cessation Advice/Counseling
